New Volunteer Application Process

Thank you for your interest in volunteering with Furry Friends. We try to make the application as simple as possible. Please review the following process.

Application Process:

  1. The first step is to print out the forms packet [pdf].
    1. If you cannot print out the new volunteer packet, we will mail you a registration packet that contains all of the necessary new volunteer forms for you and your pet (please allow 10 days).
  2. Make an appointment with your vet. The vet needs to complete two forms per animal:
    1. The Vaccination/Fecal Record (certifying that your animal is healthy and current on their vaccines)
    2. The Veterinary Pet Evaluation Form (certifying that your animal has the appropriate temperament)
  3. You will need to fill out the following forms yourself:
    1. The Registration Form (one for each family member joining Furry Friends). Please specify on this form which Etiquette Classes you would prefer. Every effort will be made to get you into the earliest possible class.
    2. The Liability Release Form (one for each family member)
    3. The Pet Behavior Etiquette Class Aggression Screen Form (only required if your furry friend is a dog and one per dog is required)
  4. Mail all of the following to our office at P.O. Box 5099, San Jose, CA 95150. Please note that this is a PO box and is not checked daily; keep a copy of all forms for your records.
    1. Five Forms:
      1. The Vaccination/Fecal Record
      2. The Veterinary Pet Evaluation Form
      3. The Volunteer Registration Application
      4. The Pet Behavior Etiquette Class Aggression Screen Form
      5. The Liability Release Form
    2. A check or money order (made payable to Furry Friends) for your initiation and evaluation fees. The amount of the check should be:
      1. $50 per person initiation fee, plus
      2. $50 per animal evaluation fee, or per human if attending with no animal

  5. Two weeks prior to the class, you will be sent a confirmation letter advising you of the date, time and location of the New Volunteer Etiquette class. This class is mandatory for all volunteers. You must bring your animal with you to class. You cannot come to a class until you have been confirmed in writing by Furry Friends. Please bring the confirmation letter with you to the class.
  6. Following the class, you will be contacted by the team captain for the first site you select. The captain will review specifics regarding that team and advise you of the first visit you can attend.
