Site Description Form

Use this form to submit a new site description or modify an existing description. Volunteers will use these descriptions to determine their interest in your site. You can describe the type of clients, the types of animals you need, any special requirements or training, and any additional information that will help them decide to visit at your site. Please do not include site addresses or visit dates and times. Below are a couple of samples below for your review. 

Examples:

Atria Willow Glen is a very nice senior living complex; it looks like an apartment complex. The grounds are nice, with lots of trees and walkways. Parking is on the street. Most of the facility is independent living. Furry Friends visits their Memory Care unit, which is in a single (locked) building in the complex. There are approximately 31 residents. The visits are held in the activities room, or, in good weather, in an outdoor courtyard.

Uplift Family Services (formerly Eastfield Ming Quong) helps children with serious emotional disturbances and their families achieve emotional and mental health. Rain or shine, this visit includes dogs of all sizes, as well as several cats. This is an Off-Leash team and dogs must have passed the off-leash evaluation class. Properly controlled dogs may be allowed off leash, to play fetch, etc. Dogs on this team must get along well with other dogs and children. Volunteers must be at least 18 years old.
